COZI is a national study to find out.
Chronic insomnia is disrupted sleep that occurs at least three nights per week and lasts at least three months.
Some signs and symptoms of insomnia may include:
✔ difficulty falling asleep.
✔ difficulty staying asleep.
✔ problems waking up too early.
✔ dissatisfaction with your current sleep pattern.
✔ your sleep problem being noticeable to others.
✔ worrying about your current sleep problem.
✔ your sleep problem interfering with your daily functioning.
Chronic insomnia is a serious health condition. Treatment can help.
Insomnia can impact your physical and mental health, safety, employment, and overall well-being.

COZI is a national research study to learn which insomnia treatments work best—behavioral, medication, or both combined
Today, doctors don’t know.
COZI is for people who:
Are willing to receive behavioral treatment, medication treatment, or a combination.
Are between 18 and 80 years old.
Have regular access to the internet.
Every COZI participant is assigned to a commonly recommended insomnia treatment (no placebos). Every participant has an equal chance of being assigned to one of three insomnia treatment groups (like flipping a coin):
COZI is designed to fit the insomnia care your doctor thinks is best for you.
If your treatment includes a medication, your doctor can prescribe either zolpidem or trazodone, and adjust or discontinue as needed.
How COZI works
1—Talk to your doctor about your sleep
Together, discuss your treatment options, preferences, and eligibility.
If you are interested and COZI is a good fit for you, your doctor will refer you to the COZI team.

2—Answer some questions online and talk with a COZI team member.
Answer questions online to confirm your eligibility.
This will take about 15 minutes.
A study team member will call you to follow up.
This will take about 15 minutes.
Read and sign an online consent form to join.
This will take about 30 minutes.
3—Begin your COZI treatment.
Before starting, answer questions online about how your sleep is now. This will take about 45 minutes.
Begin your assigned COZI treatment—
CBT-I, medication, or both.
4—Continue your COZI treatment, and occasionally tell COZI how you are doing for 1 year after joining.
Continue treating your insomnia as your doctor recommends—they are still in charge of your care!
Answer questions online about your sleep, health, and habits—1 month, 9 weeks, 6 months, 9 months, and 12 months after joining.
1- and 9-month follow-ups will take about 5 minutes.
All others will take 30-45 minutes.
Participants will be compensated up to $75 for completing the follow-ups.
COZI is enrolling nationally through participating primary care providers
COZI is focused on people with insomnia who live outside of major urban areas, where access to sleep specialists is more limited.
